To access this route, the Protagonist needs to enslave Dr. Tiff either by having Ms. Rack help out or by handing Ms. Rack the book and then tell her to share it. Speaking to Ms. Rack and telling her to share it, then speaking to Dr. Tiff and tell her to help you make more slaves unlocks this route.

Dr. Tiff: Oh... I'll get to healing you soon... Oh, Master/Mistress. You are just in time, I was just going to come get you.
Female Student: M-Master/Mistress? What???
Protagonist: So, you have a patient then, Slave?
(CG: Dr. Tiff standing next to bed and looking at shocked student)
Female Student: I-It's just a sprain! My team has a healer. I really don't need-
Dr. Tiff: I saw her limping in the hallway, trying to avoid treatment.
Protagonist: Well, sounds like she's a naughty patient then.
Female Student: P-Please! I was going to get healed, I swear!
Dr. Tiff: Think she needs... The treatment?
Protagonist: Only way to be sure she doesn't avoid future treatments.
Female Student: Are you listening? I said I was going to get healed, I swear I wasn't trying to avoid you!
Dr. Tiff: Lets begin then. I'll help with the... discomfort.
Female Student: Wait! You really don't... Ah... I don't... Did... ah...
Dr. Tiff: Don't talk back to your doctor. Now sleep.
Female Student: But I...
