I was thinking of the explanation with Reese on why changing him to male via personality change doesn't fix the issue, sorry.

For Cassandra's scene, the "cosmetic details" line refers to the timeline not the body. Like, if you were a guy and attended your best friend's birthday party, then took the sex change potion to become a girl, the only thing that would change abou the party would be that you now were female during it. It would not say, change the events of the party such that you got drunk and made out with your friend.

Cassandra's potion weirdness though basically caused that to happen; with her getting an altered timeline in which she already had dated Sarah, rather than an identical timeline where she just happened to be male.
