First encounter: Dungeon. She'll proc when you're at 20% or less total party HP.

Second encounter: Outside the school.

Third encounter: Dungeon, yet again. Just wait around for a while and a scene will trigger where she's fighting.

Fourth encounter (Charmed route): The Forest.

Fourth Encounter (Suppression route): Inside the school.
